Use the form below to enquire about an assessment, therapy or another service.
You do not need to know exactly which option is right for you. Tell us briefly what you would like help with or what you hope to understand.
Your information will be treated confidentially, and submitting the form does not commit you to booking an appointment.
Please provide a brief overview of what you are looking for. You do not need to include your full personal or medical history at this stage.
Submitting an enquiry does not create a clinician and client relationship or confirm an appointment.
The appointment structure, format, complete fee and payment arrangements will be explained before you decide whether to proceed.
